Equipment Information Form

This is to notify the Asset Inventory Administrator of a change in status or location of a specific asset.

Release for purchase of University asset

This is required for any individual or organization purchasing a University-owned asset. The appropriate University signatory can only be the Asset Inventory Administrator, Procurement Director, Associate Vice President for Business Services or the Vice President for Finance.

Release for donation of University asset

This is required for any individual or organization receiving a University-owned asset as a gift-in-kind donation. The appropriate University signatory can only be the Asset Inventory Administrator, Procurement Director, Associate Vice President for Business Services or the Vice President for Finance.

Release to transfer of University asset to an off-campus location

This is required when a tangible University asset is being transferred to an off-campus location for more than 6 consecutive months.
